This form is designed to provide Residence Life and Facilities information about your group's intended needs.  Please complete this from and return it to Residence Life by within 60 days of the begin date of your camp, or by May 01, 2008, whichever is earlier.

A deposit of 30% of the total camp price will be due 30 days before the beginning of your camp. Please see the notes at the bottom of this page for specific details.

Section A: Housing Needs for Camp or Conference Participants

Campbell Apartments are reserved for Summer School and McNair Scholars. Dobson and Blanton-Nason-Brewer Halls will be offline during Summer 2008 for renovations.

All other programs will be placed by an Residence Life Office in areas that are available and are best suited for that group's needs once registration is received.  Placement will be based on group size and available space. We will do our best to meet your first request, but reserve the right to make adjustments based on campus availability.

This form serves as your programs official registration for on-campus housing at Truman State University during the 2008 summer. If any changes are required, I must contact the Residence Life Office in 110 Baldwin Hall by phone (660-785-4227) or by email at reslife@truman.edu.
By May 1st, I need to turn in a participant list of all anticipated participants.  If am am not able to provide this, I will submit a preliminary count that will serve as our group's maximum occupancy.  Official rosters (with participant names, ID numbers, and emergency contact information) must be submitted to Residence Life no later than three weeks before my group's check-in date.  Late registrations will be accepted if rooms are available, but must be submitted no later than 3 weeks before check-in.
I understand that my program may be charged for lost keys or damages attributed to our stay.  These items will be billed to my program directly. I will be responsible to pass along these charges to any individuals, if I so choose, or absorb the expense by my program. Truman will not be responsible for this individual billing.
I understand that the Grand Total listed above is an approximation and may be adjusted based on the circumstances of my camp. For any outstanding charges that exist after the program begins, I understand that if my program dates cross the end of Truman's fiscal year, my program may be billed a prorated amount at the end of Truman's fiscal year (June 30) and the remaining charges at the conclusion of the camp. 
My group will be required to pay a deposit equivalent to 30% of the Grand Total listed above thirty days before check-in. The balance must be paid in full 7 days before arrival. If this is required of your group, you will be notified via email. (click here for information)
My camp and all its employees and participants must comply with all Residence Life policies. Also, please note that Truman is a dry campus and therefore no alcohol is allowed on campus regardless of age.  If participants or staff are found in violation of these policies the may be asked to vacate the Residence Halls or Apartments.  Residence Life reserves the right to have ResLife staff conduct rounds of the buildings and floors (at any time) to verify that all policies are being upheld.  Residence Life policies can be found in our student handbook. Informational brochures may be available to program participants, but will be an abbreviated version of our policies.
